They help keep the wrist in a neutral, level position rather than bent up off the desk, which reduces the strain that builds over a long day of typing or mousing. The best position is wrists floating level, with the rest there for support during pauses rather than constant pressure.
It comes down to feel. Gel moulds softly to the wrist and stays cool, plush memory foam is soft and warm, and firmer foam gives more support with less give. All do the same job, so choose the fill you find most comfortable.
